Registered Nurse, Hospice
Responsibilities: The person in this position is a member of the Hospice Interdisciplinary Team and reports to the Clinical Team Manager. The Case Manager, through a collaborative process, is responsible for assessing, planning, implementing, coordinating, monitoring, and evaluating the strategies and services required to meet a patient/family's end-of-life care needs. An outcome-based plan of care is created and continually adjusted through utilization of the nursing process, relationship centered care principles, and holistic philosophy, assuring that the changing needs of the patient/family are met while facilitating a pain-free, peaceful, sacred death.
Requirements: Current licensure in the State of Michigan as a Registered Nurse required. Minimum one year nursing experience, preferably in hospice or related field required. Must demonstrate understanding and commitment to End of Life Care. Must have strong Computer Literacy and have a valid driver's license.

Registered Nurse, Hospice API
